Pharmacy-First Telemedicine Platform · Live Consultation, Prescription, and Medication Delivery in One App

AppMatic Tech built a pharmacy-first telemedicine platform from scratch for a US-based telemedicine company: licensed pharmacist and doctor consultations via live chat, voice, and video, with in-app medication ordering, payment, and delivery, built in Flutter on a Laravel and Node.js dual backend.

Project Overview

AppMatic Tech built a pharmacy-first telemedicine platform from scratch for a US-based telemedicine company. The platform connects patients directly with licensed pharmacists for medication consultations, symptom assessment, and over-the-counter recommendations. Doctors are available within the same platform for clinical consultations when a pharmacist determines that a patient's symptoms require a physician's evaluation. The app handles the full patient journey from symptom input through live consultation, prescription, payment, and medication delivery without the patient leaving the platform.

Why Pharmacy-First Changes the Telemedicine Model

Most telemedicine platforms start with the doctor. The patient books a physician consultation, waits for availability, speaks with a doctor, receives a prescription, and then goes to a pharmacy separately to fill it. This workflow creates a gap between diagnosis and medication fulfillment. A pharmacy-first approach starts with the pharmacist. For the majority of non-emergency health inquiries, a licensed pharmacist can assess symptoms, recommend over-the-counter medications, review drug interactions, and advise on medication management without a physician consultation. When a patient's symptoms require a prescription or a clinical evaluation, the pharmacist refers the patient to a doctor within the same platform. The prescription is filled through the same pharmacy network. The patient completes the entire journey, from symptom to medication, in a single app session.

What AppMatic Tech Built

AppMatic Tech delivered the full telemedicine platform from scratch: a Flutter mobile app for iOS and Android, a Laravel API backend, a Node.js real-time backend, a PostgreSQL database, and a web admin panel. Every component below was engineered to carry the patient from symptom input to medication delivery in one session.

Symptom Input and Triage
AppMatic Tech built a guided symptom questionnaire that collects patient complaints, duration, severity, and relevant medical history before the consultation begins. The symptom data is structured so the pharmacist reviews it before joining the consultation rather than spending the first minutes of the call collecting information verbally. Triage logic routes patients to a pharmacist for medication consultations or flags cases that should go directly to a doctor.
Live Pharmacist Consultation
AppMatic Tech built live chat, voice calling, and video calling for pharmacist consultations. The pharmacist sees the patient's symptom input, medication history, and previous consultation records during the call. After the consultation, the pharmacist delivers medication recommendations with dosage instructions and interaction warnings directly in the app. For over-the-counter recommendations, the patient can add the recommended medication to cart and purchase immediately after the consultation ends.
Doctor Consultation with Structured Handoff
When a pharmacist determines that a patient needs a physician's evaluation, the referral transfers the full consultation context to the doctor: the symptom input, the pharmacist's assessment notes, the chat history, and the patient's medication history. The doctor receives this context before joining the consultation so the patient does not repeat their medical history. This structured handoff is the feature that eliminates the information loss that occurs in traditional referral workflows where the specialist starts from zero.
Medication Ordering and Fulfillment
AppMatic Tech built in-app medication ordering for over-the-counter products recommended by the pharmacist and prescription fulfillment through the client's partner pharmacy network. The ordering flow is integrated into the post-consultation experience so the patient moves from a pharmacist recommendation to an order placed and paid in two taps.
Appointment Booking and Queue Management
Scheduled consultations with preferred pharmacists or doctors, real-time availability management, appointment reminders via push notification, and a walk-in queue for immediate consultations when pharmacists are available online. The scheduling system handles both scheduled and on-demand consultation patterns.
Payment Processing
In-app payment for consultation fees and medication orders with support for credit and debit cards and digital wallets. Payment receipts and full transaction history are accessible in the patient's profile. The billing structure supports both direct-pay and insurance-compatible billing formats.

Technical Architecture

AppMatic Tech built the telemedicine platform on a dual-backend architecture. The primary API runs on Laravel, handling user management, appointment booking, medication ordering, payment processing, and the structured consultation record system. The real-time layer runs on Node.js, handling persistent WebSocket connections for live chat, signaling for voice and video calls, and presence indicators showing pharmacist and doctor availability. Both backends share a PostgreSQL database and a unified authentication layer. A user authenticated on the Laravel API is automatically authenticated on the Node.js real-time server without requiring a separate login or token exchange.

The mobile application was built in Flutter for iOS and Android from a single codebase. AppMatic Tech implemented the chat interface with message delivery confirmation and read receipts, voice calling with in-app audio handling, and video calling with camera toggle and screen orientation management. Chat history is preserved across sessions so returning patients and pharmacists can reference previous conversations. The call recovery engine handles network interruptions during live consultations: if the connection drops, the app attempts automatic reconnection for a configurable timeout period and resumes the session if both parties reconnect, rather than terminating the consultation.

From Scratch

AppMatic Tech delivered the telemedicine pharmacy platform as a complete system: Flutter mobile app for iOS and Android, Laravel backend for the primary API, Node.js backend for real-time chat and calling, PostgreSQL database, and a web admin panel for platform operations. The pharmacy-first consultation model, the structured pharmacist-to-doctor handoff, and the integrated medication ordering flow are the three capabilities that distinguish this platform from standard doctor-first telemedicine apps. The dual-backend architecture ensures that the real-time consultation features operate independently of the transactional API, so a spike in medication orders does not affect ongoing live consultations.

"Most telemedicine apps stop at the prescription. The patient gets a diagnosis and then has to figure out medication on their own. AppMatic Tech built us a platform where the pharmacist is the first point of contact, the doctor is there when needed, and the medication order happens before the patient closes the app. That end-to-end flow is what our users tell us they cannot get anywhere else."
